First, to address your question about R3/R4 deadline extensions: I would urge you to take these extensions with a grain of salt. Keep in mind that at this late stage in the game, many schools have already filled a large majority of the seats in their incoming class this fall. Many have also opened up the extended deadlines to applicants who were waitlisted or denied admission in R1/R2, so we have to assume that some percentage of those groups will apply again as well, and everyone will be competing for a very small number of spots at this point. You can certainly submit a couple R3/R4 applications to try your hand now, but it would be wise to still prepare yourself mentally to reapply in R1/R2 if you are keen on attending business school, because anyone's chances of admission now may not be that different from "normal."
Second, you're right that the main concern with your profile is your low undergrad GPA. You should certainly explain that you had a stronger GPA at your pre-transfer institution and that you've gotten strong grades in graduate courses since then. If you have an explanation for why your grades at your post-transfer institution dropped, you should also "come clean" and explain that to the AdCom as well (probably in the optional essay that most MBA applications have). However, at the end of the day, that was the GPA that you received your degree with, so it's fact that unfortunately can't be covered up or changed. Your 750 GMAT is definitely strong, so that's a good sign, but your best bet with your GPA is providing as many explanations as you can (without overdoing it -- just a few simple statements is enough) for why your academic performance was what it was.
