Notable achievements in highschool - 92% in 10th grade state board exams. 99th percentile in KCET test, which qualified me for medical school.
Undergrad: Top 15 medical school in India. Grades- 55% In medicine. Percentage is a poor indicator for medical degrees as only 5 or 6 in a class of 150 get distinction( above 75% marks). I'd say I stand in the 40th percentile in my class.
Work Experience: 1 year internship rotation in medical college hospital. 3 months each in the Emergency and neurology departments as a junior doctor in a tertiary care hospital. 2.5 yrs as a junior doctor in a digital health start-up. 6 months(part-time) sales and marketing intern at a VR start-up. Freelance crypto investing / trading since September 2020 till present.
Extra Curricular: Represented university in all India inter-medical university basketball tournament. Raised money for school supplies for children in orphanage as part of an NGO. Taught myself technical and fundamental analysis of cryptocurrencies to be able to live off it as a freelance trader for over a year.
Diversity: Indian, Doctor (30F)
Recommendation Letters: Can get 2-3 recommendation letters from former HoD, Professor, and Manager.
Want to attend: T20, open to European schools too, with focus on consulting as post MBA plan.
Reason for pivot from medicine - By 2nd year in med school I knew medicine wasn't for me. I'm looking for global impact, to think in terms of big picture and more analytically.
GMAT - taking the test on 28th December. It would be helpful if you could suggest me a target score based on my profile. And give me any tips to improve or fill in any gaps in my profile.

Yours is a non-typical background for a top 20 MBA program but that can be a good thing.
A few things come to mind:
1) Think your GMAT will be very important for establishing your readiness for the quantitative challenges of a top MBA program. Cryptotrading can also suggest your potential to succeed in finance type classes.
2) We need to find the leadership threads in your medical background. I've worked with lots of clients in atypical pre-MBA backgrounds and I can tell you the initial brainstorming around your past experience will absolutely clear.
3) AdCom's are notorious for not liking applicant who seem to be looking for a direction when the come to an MBA program. Your application should suggest a management path that seems logical from your past medical background (eg. Healthcare Consulting) and also be very specific about the kind of role you want and why.
For your GMAT target score, I think a 710 score would serve as strong grounding part of your application.
