Thanks for posting and for sharing a lot of background. It sounds like you know your biggest challenge (u-grad GPA) but have continued to work hard to overcome that since that point in time, which shows determination and drive and should count for something overall.

And the strong grades in your Masters will at least help alleviate some concerns about your ability to handle the coursework. You've worked your way up at work; you've done some amazing things community and extra-curricular-wise; you've got the CFA; plans for strong recs; and planning reasonably ahead.
GMAT-wise if you did do a lot better on reputable practice tests and are interested in giving it one more shot, it can't hurt. Certainly getting a score at or above the average at the schools you are looking at can only help, and more so with the lower undergrad GPA.
There's no guarantees even with some solid aspects to your profile, especially with your GPA weakness, but you're doing the right things and have overcome a lot, and now the focus is, beyond the GMAT, really nailing those application essays-- that is the place where you are able to tell your story and share who you are and what you'll bring to the class, and why the MBA and that particular school makes sense for you.
