Hi Andy,
Thanks for posting and sorry for my delayed reply. I would say that your profile is very strong but that your big weakness is going to be lack of team experience. Bschools aren't necessarily looking for candidates who have formally managed others, but they definitely want to see that you have worked in teams informally and formally and that you have led them at least informally. If you haven't had as much experience with teams in a professional setting, I would definitely try to focus on telling team stories through your other leadership experiences (volunteering/community activities or even back to college if you have to - - (that's not ideal given how long ago college was for you)).
The other things that could drag you down are your age (can't do anything about that, but something to be aware of) and your F. The "F" you'll have to explain in an optional statement. You could also take a supplementary course to prove that you are capable in a classroom setting.
I think you're "why bschool" is going to need some work as well. You don't want to say you're going to bschool because your career options in the midwest are limited -- that is not going to be compelling. Instead, you'll have to show why strategy consulting is the next logical move for you. Your long term and short terms goals also need to tie together more fluidly - they should build off one another logically. Strategy consulting to CEO is just not a believable story. So, you need to create a story that shows why you want to get to x long term goal, and how y short term goal is a requisite next step. Then you need to show why you need business school (and why that school) to get to that short term goal.
I'd say that HBS is going to be a stretch. Wharton, Kellogg and Booth are reasonable targets. Your safeties are great. You could add some schools in between like Duke, Darden etc. if you are so inclined!
