one vote for E... it is the only option where deaths do not seem to have been approved

Agreed. E is the only correct answer. The others are poorly worded and of course, one does not approve a death!

approve deaths by board of directors is funny ..

A,B,C are out.. approving deaths --wrong

D is out -- changes the meaning.. Meeting in May will also be reported in the july issue.

E is good.

Scary question.

I read it as "Promotions", "retirements", "deaths", and "other actions
approved by the board of directors at its May meeting"
will be reported in the July 15 issue of the company
paper----- and chose A as the correct answer.

But, now I see what is wrong in my reading.
