First, I just want to say I'm so sorry for the hardships you've experienced recently--that's a lot all at once. I can understand your desire for a fresh start. I sincerely hope everything turns out okay medically.
Given your age, work experience, and goals, I'd recommend you pursue an executive MBA--either at a school you mentioned or writ large. There, you'll find a lot of other established peers who are looking to expand their networks and make career shifts. It's hard for me to gauge the right set of reach/target/safety schools for you without a GMAT, and after you mentioned your GPA was average (though I'm glad to hear it isn't low and would require an optional essay).
For your extracurriculars - how recent are they? Ideally, you'd mention leadership or community experience within the last year, but ideally something that isn't too far in the past.
And regarding funding for medical situations, certain schools will provide need-based scholarships, and you would certainly be eligible.
