The correlative conjunction not only must be followed by but also. Therefore,"as well as engendering" in choice A and choice E, is incorrect.
Furthermore, has engendered is in the wrong tense. Items linked in correlative constructions should be grammatically parallel, so has engendered should be changed so that its tense matches the present tense of poses.
Choice C contains the participle engendering, which is not parallel to the main verb poses.
Answer choice D contains the pronoun they, which is supposed to refer to water fluoridation. However, since water fluoridation is singular and they is plural, choice D is incorrect.
