These are really two questions --
1) do you need a graduate degree to be accepted for a PhD program in the US
2) do you need prior US education before the PhD
My take:
1) B-schools strongly adhere to the idea that education is a good signaling device, and having a graduate degree signals that (obviously) you can do graduate level work. If you don't have that, you need an otherwise very good profile; I don't know how your GMAT and other profile pieces compare to the rest of UW applicants or other Big 10-level schools, but I would suspect that not having a graduate degree will have a greater weight on your chances of admission than not having a degree from the US.
2) I'd be very surprised if any good school restricted themselves to PhD applicants with prior US degrees. 4 years ago, I did not have that (but I had a Canadian graduate degree) and got 3 offers at very good schools for an Accounting PhD, and I'd estimate that out of all current PhD students here, a good 2/3 of them are pursuing their 1st US degree, including many in Marketing. I can't see why this'd be much different elsewhere in terms of policy.
