since the sum of even integers will always be even so its avg will always give an even integer value so 4 and if set has 2,4 as integers then median as 3

Median is the midpoint. So the median of 1, 2, 3, 4, 5 is 3.

Using only positive even numbers, can the set have a median of 1, 3 or 4?
I - no. The midpoint cannot be below the smallest number. The smallest possible number is 2. (x)
II - Yes. Were the set to contain only 2 and 4 the median would be 3 (o)
III - Yes. Were the set to contain only 4, the median would be 4. Were the set to contain only 2 and 6 the median would be 4 (o)

The answer here is D.
As the minimum number the set can have is 2, so option 1 is not possible.
Let's assume that the set is {2,4} => median = 3
When set is {2,4,6} => median = 4.
Implying that option 2&3 can be possible, thus the answer is D.
