option A & E are contenders here
we can negate B,C & D on , but as no two IC are to be connected

(E) reported, indicating that the vaccine or the vaccination program is not effective, nonetheless, no changes to polio campaigns are nonetheless is a non essential modifier which if removed makes sentence incorrect
(A) reported, thereby indicating that the vaccine or the vaccination program is not effective; however, no changes to polio campaigns have been ;use of semicolon creates clear distinction b/w IC's
option A is correct

To answer your query, Option B is incorrect because the use of "and" in place of "or" changes the meaning of the sentence.
