I think there was some error in the way you wrote out option A

A. having, for example been prescribed a drug as a dosage too low to be effective or having been

... drug "as a dosage" is incorrect... did you mean a drug "at a dosage" too low?

I went for A,
IMO, B is wordy.
C looks awkward to me.
D lacks parallelism.
E ,clear

I went for B too. For A, even with the first comma after "for example" (which seems necessary/accidentally omitted), the sentence is awkward.

EDIT: I know "being" should be avoided, as its usage is generally incorrect, but it still did made more sense than A.
