I am 4 weeks into my gmat preparations and have 2 more months left before exam. I have completed the OG quant section and my performance was average. Looking into my situation if anyone can suggest me some good book for quant , it will be really helpful. A book which has both concepts and practice would be great. Please suggest. Thanks.

If you're looking for general 'math practice', then I highly recommend Khan Academy (https://www.khanacademy.org). While the site is BIG, you should find what you're looking for if you limit yourself to basic Arithmetic, Algebra and Geometry.
Have you taken any FULL-LENGTH CATs yet? If so, then what were your scores (including the Quant and Verbal Scaled Scores)? You're going to find that there's a big difference between doing random Quant questions out of a book and handling a full, adaptive GMAT Quant section.
