I am not sure if this is how the official explanation will be done.
But I think the use of "and Where" combines both the modifiers that modifies Idaho

Lets analyzed the sentence structure

----  many people hunt both animals for subsistence, (but here there is no connector to make it parallel to first part) - it is acting as ICs

so we need need a connector which can connect properly to Idaho - in Idaho, (where) many people..................

(A), (B), and (C), lack the proper change to “where” to show there is a separate clause and (E) lacks the necessary conjunction. Only (D) gets it right: “The emergence of the disease in Idaho, which prides itself on this and where this takes place, has…”  (D) is therefore the correct answer.
