coz of parallelism and the use of "which" ; which is wrongly used here

IMO A....which modifies check's authenticity and tells how check will be authenticated.....IMO which is properly used here.

Another E. "which" is ambiguous in the original sentence. Hence, A and B are out. On the face of it, C looks attractive. But, it changes the meaning. D also changes the meaning.

OA is E, however I dont quite understand E.
Which phrase is "including...." line in E modifying? I guess, "printed instruction". If that is the case, then "including..." line is too far from what it is modifying. What do you think?
