Quote:
The school authorities recommended that the trip, which had been planned meticulously, is postponed because of continued violence in the region.

A. that the trip, which had been planned meticulously, is

B. that the trip, which had been planned meticulously, be

C. the trip, which had been planned meticulously, to be

D. the trip, which had been planned meticulously, should be

E. that the trip, which had been planned meticulously, is to be

Here school authorities are talking about the trip. Not to the trip. Therefore 'that' is required. Hence C and D are eliminated.

This sentence is using Subjunctive Mood. There 'is' is replaced by 'be'. Hence A and E are eliminated. Therefore the answer is B.
