As a former Harvard admissions interviewer and a Harvard graduate, people are always telling me their GMAT scores and asking "if I think they'll get in."
In my view, one needs a GMAT score between 680- --> 720 to be considered competitive at the "Top Ten" and Ivy League schools, and anything over 720 will put you in a completely different (i.e. excellent) ballpark and that is what you should shoot for in terms of really making a great impression with the ad com.
I have also found that in working with my own MBA clients over the years, that those who actually take an organized 6-8 week class do slightly better than those who study alone with a book or online. I have also found that scores tend to weigh just a little bit more for international candidates.
The overall goal is to make you an extremely strong candidate and not give them a reason to say no. A 720+ does just that.

You mentioned that 720+ puts a candidate in a completely different ballpark, but how big of a difference is there between scoring 760+ and 720? In other words, if someone scored a 720, but s/he could realistically score 760+ with another 100 hours of preparation, is it worth it for him/her to do so?

100 extra hours of studying, I'm not sure. An extra hour a day of studying for a few more weeks = yes. Everything counts in your application and a very high score is like any other category that is exceptional: it is going to get you noticed. If you have it in you to be exceptional - do it.
And, yes, I agree with your first statement...almost like a placebo effect -- is it the class itself, or just the fact that you're taking a class, that makes you more committed. Regardless, it is an interesting fact I've noticed.
