My answer is (D). It took me 54 seconds.

In "Not only X but also Y", the parallelism between X and Y is of paramount importance.

(A) " 2000 or more than 2000" is too wordy.
(B) "but also" should be paired with "not only".
(C) "hundreds injured" (Noun + Adjective) is not parallel with "killed ..." (Verb+Noun)
(D) Perfect parallelism.
(E) "left hundreds injured" (Verb + Noun + Adjective) is not parallel with "200 died" (Noun+Adjective).

In my opinion the correct answer should be (E)

Firstly, the given sentence specifies that hundreds have been injured and 2000 or more than 2000 are dead.

We don't necessarily need to specify 2000 twice, hence we know that atleast 2000 had died during the suppression.

Secondly, not only is usually followed by 'but also'. Hence, (E) should be correct answer.

D for me!

The suppression of the student demonstrations in Beijing's Tiananmen Square in 1989 left not only hundreds injured but also 2000 or more than 2000 dead as estimated by the Chinese Red Cross.
A) The suppression of the student demonstrations in Beijing's Tiananmen Square in 1989 left not only hundreds injured but also 2000 or more than 2000 dead
wordy

B) The suppression of the student demonstrations in Beijing's Tiananmen Square in 1989 left hundreds injured but also at least 2000 or more dead
Where is "not only"?

C) The suppression of the student demonstrations in Beijing's Tiananmen Square in 1989 left not only hundreds injured but also killed at least 2000

D) In 1989, the suppression of the student demonstrations in Beijing's Tiananmen Square left not only hundreds injured but also at least 2000 dead
CORRECT

E) The suppression of the student demonstrations in Beijing's Tiananmen Square in 1989 not only left hundreds injured but also at least 2000 died
