I am confused between "A" and "E" but i think in "E" describe is not correct to define malfunction.

So "A" i think is the answer.

I think you have focussed more on the literal meaning of "malfunction" to arrive at choice A.
Just ask yourself, what can a term do?

Note 1: A term refers/describes an action/a feature/an incident etc.
Note 2 : Whenever you see linking verbs (e.g. Is,are, seem, appear etc) ,you place equals (=) there and see if it's meaningful.

Here, A term (be it anything) CAN'T be a failure itself.

The choice A basically says that :
Malfunction = a failure to function.
But,
actually it should be
Malfunction implies to a failure to function

The term "malfunction" is a failure to function or abnormal functioning.

A. is a failure to function or Incorrect

B. can be a failure to function or Incorrect

C. describes the failure to function of Incorrect

D. is often used to describe a failure to function or Incorrect

E. describes a failure to function or Incorrect

A term cant be a failure; A/the term must always be refers to / describe something
eliminate A & B

In C -' ..........the failure to function of abnormal functioning' changes meaning and sounds awkward; eliminate

In D 'often used to ' changes meaning and redundant, so eliminate

with the word " term" i think we should use describe so, between C and E, E is the best option becuase of ''of'' in option C.
Correct me if I'm wrong.
