I read this tip somewhere before my TOEFL test and it really works! So I would like to share it with everybody who feels uncomfortable with the speaking section.
When you come to the testing centre try to register after everybody else, or at least be somewhere in the end of the queue. Why?
Because, unlike GMAT, during the break you do not have to leave the class, so you can stay and listen to others who started the test before you and already are in their speaking section. After you listen to 2-3 answers you can understand what the first and the second questions will be and prepare your answer ahead of time.
This will perfectly work for the first and second questions that are "familiar topics" questions.
Hope it will help somebody.
Good luck!
