Per Princeton Review, the answer is A. My answer is D, because I thought "which" has to modify the noun that's adjacent to "which". In this sentence, "which" doesn't modify "pre-school"

I think the answer should be D. "That" is a ambiguous, but I thought it's a better choice than A.

First of all, we can never use "which" for humans.

Hence, which in A is not referring to children.

"Which" in A is referring to "healthy community in pre-school".

Does that make sense?

Hello GYANENDRA88 , I was expecting this question from someone and here it came.

You need to understand that "which" should refer to closest noun and must be logically correct as well. It is not just closest noun rule. So, as per the meaning Pre School couldn't decrease the disease while a healthy community can. Hence, which MUST refer to healthy community to make the sentence logically correct. Does that make sense?

I'm not sure that's the case. "Which" should refer to "Programs". Vaccination Programs are what's making the community healthy. However, "Which" can also refer to School, since "which" can be both plural and singular. And last clause use subjunctive mood, so the verb is in plural form (even for singular subject). That's why I also think "which" is ambiguous.

I've read in few different places that "which" has to be adjacent to what' it's modifying.

If you look at choice E, it uses "They" instead of "which", which is also ambiguous, but better than "which" since "they" does not have to modify a noun adjacent to it.

D is a blunder here. Let me make the sentence by adding D in it.

If recent scientific evidence is to be believed, healthier community in any pre-school will be created from vaccinating younger children than those in elementary school programs; that should accordingly decrease disease in the community at large.

1. Usage of comparison the above sentence is wrong. What are you comparing? community in pre-school with community in elementary school. If so, "those" is incorrect.
2. Remember, after a semi colon you should have an Independent clause. "That" usage is acting as a restrictive modifier and this is making the entire clause dependent. This is not allowed.

Does that make sense?

the DE are wrong, i think , mostly becoz of the comparison, "younger children than those in .....", very unidiomatic, we can say this as A did, children who are younger than those vaccinated....In this way, the sentence is correctly constructed and clear.
