The pronoun ( it ) in English it can replace the word noun ( baby ) because the baby is considered unknown gender.
However, the confusing is still ongoing between the parts of the sentence
It would be = passive form, so who , what is the responsible for the action ?
Is it the hospital where the child was born ?
The word ( emerging ) = participle used as adjective describing the baby
Choice E is quite clear, so I think it is the only possible solution.

what's the source, are you sure the correct answer is option E?
I'm pretty sure the answer is option D.
Why not E? Read option E again. Pay attention to '...,because it is rated only about 20/500.'
What does 'it' refer to? the baby is rated 20/500? rudimentary sense of vision?

OK , if you think about ( which or that ) = both are acceptable modifying the nearest word ( vision )
Why I agree with E and not D
D is also acceptable, but here as if we talk about unknown baby
( as the baby , it has ) OK
On the other hand, (As) it does not talk about reason why the vision is legally considered blind for an adult.
It is because it is rated only about 20/500.
Sentence E if fully completed sentence and understood better than with D
