"efforts to" is a prepositional phrase. Hence D and E are out.
"for treating" just after "asylums" indicates that asylums are for treating in local communities. But, in fact, asylums are separate from local communities. Hence, C is incorrect.
"that" can be used neither as a pronoun (because there is no singular noun to which it can refer in this sentence) nor as a parallelism marker (because there is no other "that" in this sentence). Hence, A is out.
Answer is B because it is clear as stated.
