Hi Marty, thanks for your reply, given that these modifiers can either modify the preceding noun phrase or the preceding noun, can't we say that there is ambiguity and hence we need to cross off the answer? This is because it is ambiguous as to what the "THAT" is modifying?
No, there's no ambiguity because the meaning conveyed in each case is logical only if the modifier modifies the entire noun phrase. So, we can easily determine that the modifier modifies the noun phrase and not the noun.

In general, if a sentence read one way is nonsensical and read the only other possible way is completely logical, then we can safely presume that the way of reading the sentence such that the meaning is logical is the way it's meant to be read. Thus, in any such case, the intended meaning of the sentence is not ambiguous.
