I seem to be doing well on the diagnostic test of the OG book, getting borderline excellent/ above average in everything. I have completed the empower course and gone over my weakest points several times. I've read a couple of books to cover some weak aspects, such as word problems and grammar from books such as the manhattan books and the princeton review. Ive also completed all the OG questions from the quant and verbal review books and most quant questions from the main OG book. I really don't know where I'm going wrong. Could you please guide me on where I should go from here. Is my test date still feasible?

Your scores are in 4xx and 5xx. Assuming that you are trying to reach 700 or at least above 600, you should be working on concept building.
Your score may have plateaued because your focus is on taking mocks and seeking improvement rather than developing skills for solving questions better.
I do not know what all theoretical material you have referred to so I cannot be sure. But I think that giving mocks a break and focusing on skill building will help you.
