Frankly, I do not think the Adcom will figure it out. They take the LOR's with a grain of salt since some applicants absolutely write them or have unethical admissions consultants write those (none of these are present on GMAT Club but they exist). Of course some people have no choice as there is no concept of an LOR in their culture, so they are stuck writing multiple essays, resumes, and LORs. Anyway, I don't think it will flag anything.

Anyone else wants to take an opposite vote?

I would leave this alone, if they reach out explain the mix-up but proactively reaching out would probably more unnecessary confusion
