a question: Would D be right if i insert a comma after 'Beijing'? i reckon not as the Cause and effect is not obtained..am i right?

Unfortunately no, either of the alternatives proposed by you can't be better than the Option E. Because both the alternatives, despite of the comma, are giving the impression that its not "the city of Beijing" but "the ancient name for Beijing" was repeatedly destroyed by fire.

So, the possible correct constructions would come by placing "was repeatedly destroyed by fire" phrase before "Nanjing’s Forbidden City", so all the phrases/clauses that need to be placed together are together.
