Can someone explain why C is right and why D is wrong.

In the history of scientific and technological endeavor, there are few if any cases in which the end result was exactly as it was at the beginning.

A) as it was at

B) as it was intentioned since

C) what was intended at

D) what the inventors intended to come up with from

E) that which the inventors were intending for there to be at

A . As per meaning the sentence is conveying end result is same as it was in the beginning.
eg. you have a raw fabric in the beginning and after processing the fabric ,your end result cant be raw fabric again.
It can be something which you INTEND to make in the beginning, i meant to say the end result can be anything but not the RAW material.Here 1st option is saying end result was same as in the beginning. - WRONG

B. Simple past with present perfect (since) - WRONG
C.Its conveying right meaning without any logical or tenses error. - CORRECT
