mangamma , the word "which" properly refers to the noun phrase "20 million a year,"
which we understand to mean "20 million [newly diagnosed patients with acid disease] a year."

"Million" is a "substitute" or a pronoun in this case (same thing, just named differently).
A number can have an antecedent that is a noun phrase ("number of patients") with or without modifers (in this case, with modifiers "diagnosed with acid-related diseases").

Correct: Those chocolates displayed in the candy store window look delicious, and I would like two, please.
(May I have two chocolates from that attractive display of chocolates, please?)
