Usually lakes exist for no more than 10,000 to 15,000 years, gradually disappearing as a result of sediment.
A. years, gradually disappearing as a result of sediment. <===modifier doesn't make sense. Are '10,000 to 15,000 years' gradually disappearing?
B. years and gradually disappear due to sediments. <=== idomatic error. Replace 'due to[color=#111111]' with 'caused by' to validate. And it doesn't make sense.[/color]
C. years, disappearing gradually and resulting in sedimentation. <=== disappearing gradually and resulting in [color=#111111]sedimentation (a process). A Process doesn't fit to be an object.[/color]
D. years and gradually disappear due to sedimentation. <=== umm. looks good. No reason to eliminate. let's keep it.
E. years, and gradually disappear into sediments. <=== Grammatically don't see an error. Logically looks one. disappear into visible thing (sediments) doesn't look logically correct.
And with POE, option D is clear winner.

I thought "due to" was only a noun modifier?
