It's not correct. The correct version of the above sentence is
This indiscipline and breach of trust clearly violate the code of conduct followed by every Bank employee.
In general, if two nouns/pronouns are connected with "and" and act as subject, then correct verb form is plural. However, there are cases where the usage of "and" is considered singular. For instance-- with each and every
Every boy and girl of the class participates in the event.
