I would like to know whether highlighted portion is correct or not in above sentence.

Was this sentence from a non-GMAT source? On the GMAT, it's a good rule of thumb that 'and' in the subject means you have a plural subject, which means you need a plural verb. However, it appears that in this sentence, 'this indiscipline and breach of trust' refers to a singular noun from the previous sentence in the text, in which case things get a bit more complicated. Luckily, since each GMAT SC problem only contains one sentence, this is very unlikely to come up on the GMAT.

when conjunction and is used , always use Plural
violates is incorrect .
