Hey smano,
Well from what we see here, the biggest weakness is as you know already your GPA. If you have any way to address it, it might help you out - like if you were working full time while studying had medical problems etc. if you can give them some explanation other than "my school is hard". Your work experience is good and diverse, yet at the same time not too all over the place. The thing you will need to do here is to make sure you SHOW the Adcom that you are a shooting star - that you are amongst the best and moving up quick, and if you can do that, you should have pretty solid chances.
I can give you a relatively rough evaluation of your chances, but I really need more info (about your leadership achievements), but based on your goals, work experience and numbers I'd say Wharton/Chicago/NIT are stretches. You should be relatively competitive at the other three on your list. which means that going forward, to fill out your school selection strategy, you might want to think of adding one or two "Safe schools" to your list.
