(A) A little hope that there was of reviving the patient from coma evaporated when his blood pressure level began to drastically fall.
(B) A little hope that there was of revival of the patient from coma evaporated when his blood pressure levels began to fall drastically.
The hope ...that was is correct as compared to A hope ..that was. So we can cancel A & B.
(E) The little hope which the patient had of being revived from coma evaporated when his blood pressure level began to drastically fall.
E is incorrect as it says the Patient had hopes of being revived.
(C) The little hope that there was of reviving the patient from coma evaporated, when his blood pressure level began to fall drastically.
(D) The little hope of reviving the patient from coma evaporated when his blood pressure level began to fall drastically.
C is wordy compared to D. Also use of comma in C is questionable.
D is a better option.

The difference between "a" and "the" is that "a" is an indefinite article and "the" is a definite article. Here we are talking about the hope of reviving the patient and not hope in general. Hence "The" is correct. So A and B are out.
E changes the intent of the question.
"The little hope of" is better than "The little hope that there was" since the latter is wordy. Hence C is out.
D it is
