I don't see a problem with "to do". According to the MGMAT Sentence Correction guide I'm reading, when the parallel structure contains 2 verb infinitives, the second "to" is optional. English is not my mother tongue, so it doesn't sound wrong to me as it may do for you, but I don't see the problem if they chose to insert the "to" here. Probably they just wanted to confuse as much as possible, because C is the only answer with "it" at the end.

'that' can not be used standalone unless it modifies the noun next to it.

Eg. That book, that boy..... But "it" can be used at any one part of the sentence as long as it maintains correct pronoun reference.

In the above two sentences, the second construction has issues with parallelism. Eliminate D and E.

B is a nonsensical construction ending the sentence awkwardly with "achieving such". Achieiving such what?

A has a major reference issue. The option uses 'that' to refer to "reform", whereas we should refer this singular noun with 'it'.

C corrects all the mistakes discussed above and is hence the right choice.
