IMO C

(D, E) change the meaning of the sentence.

(A) "cost of", not "cost with"

(B) who is "making beneficiaries pay"?, Medicare or nonhospital services?

should there be a "the" infront of beneficiaries?

D & E are out because WHICH should refer to Medicare and here it refers to home health care. 
A is not right because "but not with" seems wrong
B is out because "Making benefeciaries" in B is not clear what it modifies. 

So C remains.

logic for not choosing A - Is where not very awkward in this sentence? Where is used for places.

B. but not of other nonhospital services, making beneficiaries pay 20 percent of the costs.
Logic for not choosing B -hospital services cannot make beneficiaries pay money.

Does it makes sense?
