The record for is correct idiom here. A record of means a note of - take a record of class attendance.

Option E is missing the the word "women's" before singles titles. All other options have it. Wouldn't that change the original meaning of the sentence.

option E is the correct choice. But, what about the missing "women's". Crack700 correctly pointed it out. Even i have the same query.

SHE is not needed here.

I agree with Financier.
This SC is weird, please provide the sourse of the question. I don't think this is from OG.

I have never ever seen that construction not only ..... but [] also to have some words between but and also and to be correct.

I am no expert, but I believe "Not only... but she also..." is not wrong.
I think the answer is D because it is the only answer where I did not see anything "wrong" (please correct me if I am wrong)
I think E uses ":" incorrectly, as someone stated above, and C looks poorly constructed. Its questions like these that ruin a perfectly flowing GMAT verbal test :P. What is the OA and explanation?

I think the part of the sentence before the ":" has to be complete.
For example, srjaidev is going to get dinged from all the schools he applied to: HBS, LBS, Stanford. :-D
Another example is - she holds a record at Wimbledon: most women's singles titles won.
