I have never ever seen that construction  not only ..... but   also  to have some words between  but  and  also  and to be correct.

I am no expert, but I believe "Not only... but she also..." is not wrong. I think the answer is D because it is the only answer where I did not see anything "wrong" (please correct me if I am wrong) I think E uses ":" incorrectly, as someone stated above, and C looks poorly constructed. Its questions like these that ruin a perfectly flowing GMAT verbal test

I think the part of the sentence before the ":" has to be complete. For example, srjaidev is going to get dinged from all the schools he applied to: HBS, LBS, Stanford. :-D

Another example is - she holds a record at Wimbledon: most women's singles titles won.
