The only thing that can be done now is ask your recommender to send an email to the admission committee to consider the new document stating what has been changed and provide some sort of justification for the same.
I don't think there is a way to edit the recommendation form now.
You can do one of my least favourites: If you have not submitted the application yet, delete his/her name from the application portal and then add the information again. It may trigger a new link. Note: I have never tried this and would be very cautious to do so.
