It says here "We can leave out that wherever it may appear in a sentence but not if it is at the beginning. "
Correct, Schachfreizeit. That resource mentions that the conjunction ‘that’ may be left out without affecting the meaning of the sentence.
But, the GMAT doesn’t like to leave out the ‘that’. So… we have to play by the GMAT’s rules.

Is this a general rule in the GMAT? So whenever a that is missing, the sentence is wrong?
Unfortunately there’s no such thing as general rules when it comes to the English language, even on the GMAT. The language evolved organically over many centuries, so it’s full of idioms, exceptions, and inconsistencies.
Having said that, if you’re looking at two answer choices in SC, one that’s omitting the ‘that’ and one that isn’t, go with the one that isn’t, Schachfreizeit.

The teacher is confident her students mastered the lesson

I think this sentence comes from target prep.
It said there, that this sentence is not correct since a "that" between confident and her is missing.
"not correct" might be a bit too extreme.

An official sentence:

Many experts regarded the increase in credit card borrowing in March not as a sign that households were pressed for cash and forced to borrow, but as a sign that households were confident they could safely handle new debt.
