A rectangle PQRS contains a circle C, with center O such that C is the largest possible circle which can be inscribed inside PQRS without crossing the sides of this rectangle. Is PQRS a square?

(1) OP = OR

(2) OP = OQ

Both statements tell us that the centre of circle is equidistant from four vertices..

But the same can be both the rectangle or square. See attached figire
