Perhaps their indicates that the trained inspectors are part of Olympic Foods. Otherwise it is good. A

B. revealed that the beef products were contaminated with Salmonella bongori, types of Salmonella that were
Salmonella bongori is the species. Contamination would need to be by multiple species for it to be types of Salmonella. A

C. reveals that their beef products were contaminated with Salmonella bongori, a type of Salmonella that is
The examination was conducted. It happened and now the results are being revealed. A

Revealed that is correct so A and D are out.
In C, use of "is" wrong.
In B, with Salmonella bongori is singular so use of were in modifying phrase "types of Salmonella that were" is wrong.
D is correct.

However, "considered to be" in non underline portion is wrong.
I think question maker doesn't make such error in non-underlined portion.

IMO E. usage of tense ' conducted/ trained/ revealed, meaning stands out well in E
in D subject ' examination' is singular and the verb in D ' reveals' is singular so it would be correct but after reveals ' that' is missing which makes D option in correct over E .. where as in A usage of 'their' is ambiguos as not sure what it actually refers to beef products or trained inspectors..
in B 'types' usage is wrong...

IMO E should be correct..

Official Explanation:

there are simple grammatical rules we can follow. Parallelism – because the examination was “conducted” (past) the verb “reveal” should be in past tense as well (revealed). The verb reveal should be followed by the word “that” because it introduces a new section describing what the examination revealed. The use of the word “their” is ambiguous, as “their” can refer to the inspectors or Olympic food, and therefore “their” should be replaced by the word “the”.
