They and them in A, B, and C options are ambiguous because of having two nouns
A. Grocery Stores
B. Mix Salads
We don't know for sure about which noun fixes well here.
Eliminate A, B, and C.
Between D and E.
In option E, After having been contaminated....
What is contaminated? Grocery store or salad? Still ambiguous.
D particularly states the contamination is done in the product (salad) (No ambiguity)
Answer is D

The non-underlined portion starts- with several grocery stores.
So modifying clause (present before the COMMA) should modify the stores and also portray clear meaning.
In A, B, C, and E all portray the meaning as if stores are being contaminated with the E.coli bacteria which is wrong it's the product that has been contaminated.
Only D removes that problem.
