This is the response that I got from Admissions (may be useful to you)
In order to qualify for the reduced application fee please complete the Reduced Application Fee - $125 Fee section located at the bottom of the Application Fee Waiver Information section of the application. You should mark "yes" in response to the question "Are you qualified to receive an application fee reduction?" In the box, please write "Campus Visit, October X, XXXX (insert date)."
Once you click save at the bottom of the page the fee reduction request will be sent to us. You will receive an email once the reduction has been approved. Once you receive the email you will be able to submit your application at the reduced fee rate.

I listed all my schools. There has been debate on how to handle this. I do believe the field is optional so you can leave it blank. At the end of the day, my view is if they reject me because they think I will go to another school, then so be it. It gives me hope that I will get into the other schools
