I think the answer is (C)
(A) subject of the sentence should be "the teacher", not "a pop quiz", because we have "Disappointed by ..." at the start of the sentence.
(B) subject of the sentence should be "the teacher", not "they", which may refer to the students
(C) Correct as mentioned above
(D) here the sentence means that the students surprise the teacher with a pop quiz, meanwhile the correct meaning should be the opposite, that the teacher surprises them with a pop quiz. This answer choice has wrong meaning, so it is eliminated.

(A)Here phrase "Disappointed by his students' refusal to complete the assigned reading" is modifying " a pop quiz". This doesn't make sense. So option A is wrong due to modifier error.
(B) Here the phrase "Disappointed..." is modifying "they were". Hence, option B is also wrong due to modifier error.
(C) Here the phrase "Disappointed..." is correctly modifying "the teacher". This sentence makes sense as well. So C is the correct answer.
(D) Here modifier error is corrected. But the sentence does not give correct meaning. Hence, option D is wrong.
(E) This option also has modifier error like first two options.
