This question is little redundant, choice A seems to be the most errorless choice among all.

Can anyone say why D is incorrect

Option D alters the meaning of the original sentence. The original sentence stresses that the invention of penicillin by Dr. Alexander Fleming and his team directly caused a decrease in deaths resulting from illegal abortions.

However, option D shifts the emphasis to the fact that fewer women died after the invention of penicillin due to illegal abortions. This change in emphasis takes away from the clear cause-and-effect relationship between penicillin and fewer deaths that the original sentence intended to convey.

As a result, option A is more effective than option D because it communicates the intended meaning in a straightforward and unambiguous manner.
