Aah - now this makes sense. egmat I agree with guddo fully. The public being unaware is neither the obstacle nor what the correct choice is truly focusing on. The correct choice mentions it simply as a modifier to describe "hazards".

But guddo is spot on. It is the pitfalls and uncertainties themselves which are the potential roadblocks. Thanks guddo

I’d say the obstacle in (E) is not the public’s unawareness itself. The obstacle is the pitfalls and uncertainty that the public does not yet fully appreciate.

The passage says gene therapy has great potential, but there are hazards and uncertainties ahead. Those hazards could block or delay mainstream acceptance once they become clearer.

(D) is only one example showing that the field is still uncertain: only one controlled study has been published. But (E) captures the broader obstacle the author points to: unknown or underappreciated risks surrounding gene therapy.
