Do B-School applications ever ask how many hours a week you spend on work / extracurricular activities?

Ex:
-65 hours a week for employment
-10 hours a week for extracurricular activity 1
-5 hours a week for extracurricular activity 2

etc

Yes. I can't recall which schools, off the top of my head, but at least a couple of mine did ask for hours/week commitments. (Maybe Wharton? Columbia?) And since a couple of mine did, it's looking like a couple of yours will... :)
