Start practicing 5-6 RC questions per day, without time to clearly understand , then you can start with time

I have gone though the GMAT Ninja videos, but making short notes of RC paragraphs is hard for me—it takes a lot of time, and I feel I do better without them. I am able to solve medium level RC with 80% accuracy, but I'm really struggling with the hard ones. I am using the Official Guide Verbal Review to practice.

Try doing a session or two with a study buddy who is really strong on RC. May add a fresh perspective to see how someone else goes through a passage and different question types. You could offer to help with an area of the GMAT you're strong on in exchange. Searching for/working on a reading methodology that promotes absorption could also be helpful.
