I'm noticing a strange trend. When I get questions wrong, I more than often get two questions wrong in a row, while practicing the OG as well as in mocks. This happens in questions of all range, from easy to difficult and even in mixed difficulty. Is there a reason this could be happening?

Sometimes we keep in mind previous questions and try to solve it. Therefore do not concentrate on current question on 100%.
