I have a theory

the split between 'who were healthy' and 'those with better record' can be explained by focusing on the tense 'were'.
'were' cannot be parallel to adapted;it should be parallel to 'sickly' to match the timeline.
Hence usage of 'were' is wrong.

makes sense?

(A) had been sick as children adapted better than those with a record of good health.
Sick isn't the right usage as it's just used to identify a trait

(B) had been sickly as children adapted better than those with a record of good health.
This nails the reasoning and the tense let us hang on to iy

(C) had been sickly as children adapted better than those who were healthy.
who isn't right usage it provides ambigous meaning

(D) had been sickly as children adapted worse than those with a record of good health.
better should be the right usage not the other way round this completely twists the meaning

(E) had been sick as children adapted better than those who were healthy
Similar reasoning as C
Hence IMO B
