They have to write a general letter of recommendation that needs to be uploaded (in PDF/ word format) on the given link. Apart from that there will be a small form where the referee need to fill in a few details and rate you on various skills like communication, leadership etc.

Honestly I didn’t see anything mentioned about it but just to be safe I added both a letterhead and attestation/signature. But I did get an acceptance so it worked out for me.
