Most of the Top 30 schools have the requirement.
That said, if you are able to demonstrate that English was the sole med of instruction at your undergrad, you may be eligible for a waiver

Yes, each school has a minimum TOEFL score/language requirement. They do range from school to school, so make sure you visit the admissions office website for each program on your list.
If there's any ambiguity about placing out, you should email or call the admissions office of each program to clarify. You should be extremely specific so you don't encounter this problem near or after the submission deadline. Email is better so you have it in writing.
